Meh, saw this request a few times now, so I transcribed it; pretty good piece anyways =)

It's for a small ensemble. At one point I improvised the piano part a bit because it was difficult to decipher it from the guitar, but I don't think it'll be that big of a problem.

If you want to play it on the piano, you could try replacing the non-melody piano parts with the melody from violins, and guitar for harmony, maybe...
