I swear I've scoured the net for this (even on japanese sites!) and all I've found is one site that had a corrupted file that supposedly had this sheet music in it. I've requested that they fix it several times, with no reply, so here I am.

It was originally the ED for a Game called Routes, and the Arrangement for this is by Junya Matsuoka (I don't know who the original composer was)
