There is no specific title for the song, but I'm pretty sure it's the music played on the dying minutes of the last episode. However, there is a better, extended version of this, and I'm dying to have the music sheet for this one (if possible, piano + violin). Besides, there aren't any sites out there with the piano + violin transcribed into one.

Again, the anime is Myself; Yourself played by Yatsushiro Nanaka (anime character).
