Ah, the arrangement by Marasy (まらしぃ).

I keep an eye out for sheets posted on Nico Nico and some of his stuff has come up, but none yet for From Y to Y.

In the meantime, I did find a band score at http://jeun0etude.blog111.fc2.com/ (forget which post exactly). It's a Guitar Pro file though, so I extracted the piano + melody part and made a pdf. http://snomits.tumblr.com/post/992798946 for details.

Hope that's at least a bit of help... and that someone does transcribe Marasy's version.
