My immediate thought: such a famous song must have already been transcribed many times.

Anyway, I might transcribe one based on the anohana ED version, for piano and voice. (I won't make one for piano solo since it has already been done.)

Is the song sung at a different key to the 10 years after ver.?

10 years after Ver. is in F#. I don't have the original one with me, but I think it is in the same key.

One of the piano arrangements at print-gakufu shifted it to G to make it easier to play.

Edit: By the way, I am nearly finished with a simple TV size for piano and voice.

A few lyrics correction to your melody score:
bar 3 (and similar): きぼう for 希望, rather than きおう.
bar 9 etc: さいこう for 最高, not さいごう. (i agree it sounds like that...)
bar 28: いっしょ for 一緒, not いっしょう.
bar 31: てれくさそう not てれくささ そう.
bar 125: ほほ for 頬, not ほう.
bar 135: に not い.
