Do you want the chords or the strings?
E, F, A, C, Db, G for the strings. As for piano chords, you simply harmonize it any way you like (don't worry, there's no tricky chords, just common chords), which I hope you know what major, minor, dim... chords are. Use the bass to aid you. I wrote a transcribing tutorial on this; See how to transcribe thread. Plus I don't like transcribing off of youtube. It's difficult to hear stuff in the background.
