There is a piano solo for sale at http://www.print-gakufu.com/score/detail/51376/ ; there is also a sample where you can see about half the notes.

The accompaniment is based on Pachelbel's Canon, so you might get an idea of the chords from scores of that.

By the way, I planned to transcribe this for piano+voice a few years ago, but still haven't got to it yet... Not sure when I will do it. :P

Edit: In fact, this song is just repeating 4 bars over and over with slight variations and different instruments (except the middle section).
Finished the whole song for piano+voice. I will tidy it up in the next day or two and upload it.
Not sure if I will make a piano solo version afterwards... (It is quite easy for this song: just play the vocal part with the right hand and do broken chords or some patterns of the chords on the left.)
