Bleach - Going home

This is a famous guitar piece has touched the hearts of many of its viewers through many episodes throughout the years. However, the other day I crossed this piano version on one of the filler episodes and fell in love with this version. The piano version is different from the original... but not. Played differently but still has the same feel and ambiance.

I've looked everywhere for this variation of the piece but couldn't find it. I'm positive that one of you trascribers can easily re-create this masterpiece using the chords from the guitar sheets. Thanks.
