It is from the PC game version of Da Capo, which was both an excellent game and an amazing anime.
I've been searching to the extent of my ability for sheet music for this song.
A search came up for this website, but unfortunately, it was for a comment in the "Transcriber's Status" page, and a file was not present.
The person that posted that comment said that it was "simple", so I'm sure that the more experienced people here would be able to do it with no problem.

Sheet music for this song would be very greatly appreciated. I love this song, and I have been listening to it for years now.
